Eganville Minor Hockey is seeking members to coach and mentor youth on our House League teams. Please complete the below form to submit your application for the 2023/2024 season.

Qualifications
You do not need to have all the qualifications to apply for a head coach position. The selected candidate will be required to complete all necessary courses within a specified timeline. You can identify which course are required by visiting the Bench Staff Qualifications link. EDMHA will reimburse bench staff members for completing the necessary qualifications on a prorated basis. Contact an Executive member for more details.

All minor hockey coaching staff members are required to have a valid criminal record check with vulnerable sector screening. If applicants do not have a current CRC they may still apply however must immediately submit to the OPP a request for the CRC be completed.

By submitting this application I verify that I am aware of and will support the main objectives of the Eganville District Minor Hockey Association. Should I be entrusted with the opportunity to work with a team, my conduct will be exemplary and I agree that any behavior on my part or my staff that is contrary to the Hockey Canada Code of Conduct for Coaches could result in a loss of coaching privileges.
To be considered as a Volunteer candidate all applicants must understand and agree to the following conditions:
1. Accept full responsibility for the team conduct, finances, team officials, etc.
2. Operate within the EDMHA Coach Code of Conduct.
3. Complete a Police Record Check with Vulnerable Sector Screening that returns with a no record result.
4. Agree to verification on any information provided on this application form.
5. Agree that all coaching qualifications are complete prior to Hockey Eastern Ontario deadlines.
